Worm Cultists are agents of the Worm Cult. A trio of Worm Cultists will appear and knock you out after you speak with the Benefactor, and initiate a cutscene in which you witness your demise.

You can also witness a couple of Worm Cultists escaping with Thallik Wormfather during Shattered Hopes if you get to the central ruins before they enter into the portal.
